1.泛硫乙胺:
泛硫乙胺是泛酸(维生素B5)的衍生物,可作为维生素的来源。泛硫乙胺及其代谢产物似乎对人体的脂肪和胆固醇代谢途径起作用。维生素B5的一个显著功能是其转化为辅酶A,辅酶A是脂肪酸代谢为细胞能量的必要因素。泛硫乙胺衍生物半胱胺还可降低产生胆固醇和甘油三酯的肝酶活性1。研究表明,在高胆固醇人群2,3和糖尿病患者4中,每天服用900-1200mg的泛硫乙胺可显著降低总胆固醇和LDL胆固醇(高达13.5%)、甘油三酯和升高HDL,尽管在低至600mg/天的剂量下也观察到对甘油三酯的显著影响5。一项控制良好的临床试验表明,当将泛硫乙胺添加到低饱和脂肪和低胆固醇的饮食中时,可进一步降低总胆固醇和LDL胆固醇6。

3.益生元:

4.辅酶Q10:

5.维生素E:

6.膳食纤维:

7.大蒜:

大蒜还能降低高血压患者的收缩压和舒张压,以及血压正常的患者的收缩压。对11项大蒜安慰剂对照人体试验的回顾和分析显示,大蒜组的收缩压平均下降4.6mmHg;高血压亚组的收缩压平均下降8.4mmHg,舒张压平均下降7.3mmHg36。
8.绿咖啡(绿原酸):
未经烘焙的咖啡豆提取物或生的绿咖啡豆提取物已被用于促进心血管和代谢健康。绿咖啡含有高水平的多酚,称为绿原酸(CGAs),这些多酚被认为是咖啡中重要的健康促进化合物。然而,传统的咖啡烘焙方法通常会降低多酚水平37。临床前研究表明,CGA可能通过促进胆固醇7α羟化酶(CYP7A1)的表达增加来帮助调节胆固醇水平,CYP7A1是负责催化胆固醇转化为胆汁酸的第一步的酶38。在小鼠模型中,降低血浆总胆固醇和LDL胆固醇水平,促进巨噬细胞排出胆固醇,这一过程有助于防止动脉粥样硬化斑块的形成。它还降低了甘油三酯和一些炎症生物标志物39。
在健康人群和心脏代谢疾病患者中进行的大量临床研究已经证明了绿咖啡提取物对脂质水平以及其他心脏代谢标记物的益处40。在一项对14项随机对照试验(包括766名参与者)的荟萃分析中,剂量范围为90至6000mg/天(提供13.5至1200mg/天CGA)的绿咖啡提取物达16周可降低血清总胆固醇。在持续八周以上的研究中,LDL也显著降低41。在一项对包括992名参与者在内的27项随机对照试验的荟萃分析中,类似剂量(200至2000mg/天)的绿咖啡提取物可持续24周,降低血清甘油三酯,提高HDL水平42。绿咖啡提取物似乎对葡萄糖和胰岛素水平、体重和血压等也有积极影响43-45。
9.茶多酚:
多酚是一种多种多样的植物营养素,在饮食中无处不在。绿茶和红茶都含有多酚。在几项研究的分析中,绿茶儿茶素(EGCG)表现出显著的胆固醇(LDL)降低(四项研究平均约为9mg/dl)46。一项荟萃分析证实了绿茶对总胆固醇、LDL以及收缩压的有益作用47。在有患糖尿病风险的肥胖女性中,绿茶与糖尿病药物二甲双胍的头对头比较发现,绿茶在降低总胆固醇和LDL方面表现更好48。随后,一项对47名轻度高胆固醇症的日本男性和女性进行的红茶提取物研究表明,三个月后,总胆固醇降低了8%,LDL胆固醇降低了13%49。在一系列临床前研究中,红茶中的茶黄素有效地降低了总胆固醇、LDL和甘油三酯50。
10.姜黄素:
姜黄素是姜黄根的主要活性成分,在心血管疾病中具有多种保护作用,可能减少氧化应激、炎症以及平滑肌细胞和单核细胞的增殖。95项小型人体试验揭示了姜黄素对降低脂质过氧化的作用51,52和血浆纤维蛋白原53,二者都是动脉粥样硬化进展的因素54。姜黄素也可以通过增加LDL受体的产生来降低血清胆固醇55,56。
但是,尽管在动物模型中取得了成功,关于姜黄素抗高胆固醇血症作用的人类数据存在矛盾。一项针对10名健康志愿者的小型研究显示,在连续7天每天使用500mg姜黄素时,脂质氧化产物(-33%)和总胆固醇(-12%)显著降低,同时HDL胆固醇(29%)增加57。在随后的两项研究中,低剂量姜黄素在急性冠状动脉患者中显示出降低总胆固醇和LDL的非显著趋势58,而高剂量姜黄素(1-4g/天)的总胆固醇、LDL和HDL胆固醇表现无显著增加59。
对七项随机对照试验的综合分析得出结论,姜黄和姜黄素在降低LDL胆固醇和甘油三酯方面有效,没有任何严重的副作用60。在一项针对2型糖尿病患者的研究中,与安慰剂相比,补充姜黄可降低体重指数、总胆固醇和甘油三酯61。一项涉及代谢综合征患者的类似研究发现,姜黄素对脂质参数(降低LDL和增加HDL)有有益的影响62。
11.红曲米:
红曲米是由红曲酵母发酵而成的传统米制品。酵母产生的代谢产物(Monacolins,莫那可林)是天然存在的HMG-CoA还原酶抑制剂(其中一种,Monacolin k在化学上与洛伐他汀相同)63。一项对包括近10,000名受试者在内的93项随机试验的全面综述表明,红曲米的商业制剂降低了总胆固醇、LDL胆固醇、甘油三酯,以及HDL胆固醇的增加64。一项多中心、良好对照的试验中,将一种名为“Xuezhikang(血脂康)”的红曲米提取物与安慰剂进行了比较,结果表明LDL降低了27%65。一项对22项临床试验的综述表明,在冠心病的常规治疗中加入血脂康,可以降低死亡和心脏病发作的风险66。

20.香柠檬:

香柠檬还可以补充他汀类药物作用。一项开放标签安慰剂对照试验招募了77名LDL胆固醇和甘油三酯升高的患者,并将他们随机分为四组:1)安慰剂,2)每天10 mg或20 mg瑞舒伐他汀,3)香柠檬多酚,4)香柠檬多酚加10 mg瑞舒伐丁。研究人员发现香柠檬多酚增强了罗苏伐他汀的降脂作用77。
21.余柑子(印度醋栗)
印度醋栗(Amla)是余柑子(emblica officinalis)植物的俗称,传统上在印度阿育吠陀医学中用于多种疾病78。在动物模型中,印度醋栗表现出强大的抗氧化和抗炎作用79。例如,在高果糖饮食诱导的代谢综合征动物模型中,余柑子提取物的给药控制了胆固醇和甘油三酯水平的升高,并显著降低了代谢综合征中一些炎症相关基因的表达80。
几项临床试验表明,余柑子提取物对血脂和胆固醇有良好的影响。健康和糖尿病志愿者连续三周每天服用2或3g余柑子后,总胆固醇和甘油三酯显著降低79。在60名高脂血症患者中,余柑子和辛伐他汀(Zocor)的头对头比较显示,对血脂的影响相似81。在2型糖尿病患者中,与基线和安慰剂相比,每天服用两次250或500mg标准化为60%低分子量可水解单宁的余柑子提取物,可显著降低总胆固醇、LDL和VLDL胆固醇、甘油三酯,并显著提高HDL胆固醇水平82。在一项为期12周的试验中,15名超重或肥胖的成年人平均年龄为36岁,每天两次500mg,这种相同的高单宁提取物对心血管风险标志物具有显著的有益影响。与基线相比,余柑子提取物降低了计算的LDL胆固醇、总胆固醇/HDL比率和hs-CRP水平83。
22.关于烟酸与血脂管理:

1.泛硫乙胺:
泛硫乙胺是泛酸(维生素B5)的衍生物,可作为维生素的来源。泛硫乙胺及其代谢产物似乎对人体的脂肪和胆固醇代谢途径起作用。维生素B5的一个显著功能是其转化为辅酶A,辅酶A是脂肪酸代谢为细胞能量的必要因素。泛硫乙胺衍生物半胱胺还可降低产生胆固醇和甘油三酯的肝酶活性1。研究表明,在高胆固醇人群2,3和糖尿病患者4中,每天服用900-1200mg的泛硫乙胺可显著降低总胆固醇和LDL胆固醇(高达13.5%)、甘油三酯和升高HDL,尽管在低至600mg/天的剂量下也观察到对甘油三酯的显著影响5。一项控制良好的临床试验表明,当将泛硫乙胺添加到低饱和脂肪和低胆固醇的饮食中时,可进一步降低总胆固醇和LDL胆固醇6。

3.益生元:

4.辅酶Q10:

7.大蒜:

8.绿咖啡(绿原酸):

9.茶多酚:

10.姜黄素:

22.关于烟酸与血脂管理:

医疗干预
通常用于降低高甘油三酯药物如下:- 贝特类调脂药:非诺贝特、苯扎贝特和吉非罗齐等,通过降低肝脏极低密度脂蛋白(VLDL)胆固醇的产量和加速从血液中去除甘油三酯来降低甘油三酯。 VLDL胆固醇主要含有甘油三酯。
